I hereby register my interest in participating in MWI’s Sustainable Tote Delivery Program. I understand that this will include receiving my orders in reusable totes on a consolidated delivery schedule (typically twice a week). I further understand that completing this form does not guarantee my participation in the program. I acknowledge that MWI will notify me when a route is available in my area and will determine my consolidated delivery days based on courier availability.
